Mrs. Ann E. Culver
In reply to your note of the 23rd of July 1862.
I have to inform you that your husband Parmenus Culver was admitted into this institution on the 23rd of June 1862 suffering from flesh wounds of the hip, of which disease he died on the 3rd of July 1862.
Enclosed you will find a pocket book containing 56 Cts, also his descriptive list.
